For Your First Visit

Please arrive at the appointment time you were told by our staff. In addition to the completed forms in your welcome packet (if there was time for one to be mailed to you), please bring the following items with you to your appointment:

• Current insurance and prescription cards, and photo ID
• Your current medications (including over the counter and herbal medications) – please bring either a list from your pharmacy or the actual bottles
• Any records, films, and/or lab reports you may have related to your visit
